Concrete pads are a separate scope. The provider removes the structure, but a concrete pad requires different equipment and disposal — concrete is very heavy and breaks differently than shed materials. If your shed has a concrete pad you want removed, call to discuss and the provider will quote accordingly or refer you to a concrete removal specialist.

Do you remove the shed contents, or just the structure?

Both options are available — structure only or structure plus contents. Contents are priced separately by volume on top of the flat-rate shed removal. Mention what the shed contains when you call so the provider can plan the job correctly.
